Just seconds before a building is demolished, a mysterious mute child that has been living alone underground is discovered. When the FBI's Fringe Division investigates, the boy forms an unspoken connection with Agent Olivia Dunham. Meanwhile, an especially gruesome serial killer resurfaces and showcases his "artistry" by displaying his work publicly. As the boy gradually assimilates into a new environment, Olivia and the team must race against the clock to prevent the mad man from further macabre in Inner Child.

Construction workers are rigging a building for demolition. Upon completion of their work, two workers walk out of the building when one stops silently in mid-stride. When queried by is colleague, he starts back to the building, stating emphatically that they have forgotten something. The pair return to the building and the one worker convinced that something is amiss is seemingly drawn to a small landing. Stomping on the area, the worker nearly falls into an opening in the floor as it crumbles, revealing a sealed in area of the building....an area apparently not on the plans. They radio to delay the demolition and are joined by a third co-worker and together enter into a dark tunnel. The tunnel/chamber is full of aged and decayed debris, with rats crawling through unabated. As their search lamps pierce the dusty darkness, they see what appears to be metal bars with a door-like opening on one side, akin to that of a prison cell. As they proceed in, they eventually direct their search lights into the corner of the room where a small, bald, and pale child stares at them panting.
